To receive credit for a SMAD internship, you need to complete this entire application form. Once you have submitted this application, the SMAD internship coordinator will review it. If approved, you will receive further instructions on how to register for SMAD 495.

In addition to this application, the internship coordinator must receive an email confirmation from your supervisor verifying that you have been accepted as an intern and will be completing work with the company.

You can receive 1, 2 3 or 4 credits for your internship. The number of credits is based upon the minimum number of hours you need to complete.
